About the role:
Your duties and responsibilities will include, but are not limited to:
- Conduct support needs assessments by gathering and evaluating information from participants, representatives, and support networks.
- Analyse and interpret information to make evidence-based decisions regarding support requirements.
- Prepare clear, accurate, and comprehensive assessment reports
- Apply policies and procedures using sound judgement while maintaining a participant-centred approach.
- Communicate effectively with participants, families, and stakeholders, explaining assessment processes and outcomes clearly.
- Identify and escalate complex issues, risks, or policy concerns to support timely resolution.
- Collaborate with colleagues and stakeholders to support planning outcomes and continuous service improvement.
- Use digital systems to accurately capture, manage, and maintain information in line with privacy and security requirements.
- Adapt assessment approaches to meet individual needs across a range of settings and circumstances.
- Support team capability through knowledge sharing, mentoring, and collaboration.

Mandatory Qualifications:
Recognised tertiary qualification in allied health: (evidence required with application)
- Occupational Therapy
- Physiotherapy
- Speech Pathology
- Social Work
- Dietetics
- Exercise Physiology
- Rehabilitation Counselling
- Counselling
- Psychotherapy
- Registered Nurse or Enrolled Nurse
- Clinicians with an Education degree
- NDIS Behavioural Support Practitioners
Desirable skills and experience:
We strongly encourage your application if you have demonstrated the below skills and capabilities:
- Experience in the administration of assessments of support need, or other relevant person-centred assessment frameworks, with a strong commitment to person-centred and strengths-based approaches.
- Accreditation or experience using the I-CAN Support Needs Assessment Tool is highly regarded, but not essential.
- Passionate about working within the disability sector and positively improving the lives of individuals and their families.
- As part of the eligibility and suitability requirement, only Australian Citizens need to apply
- Willingness to undertake pre-employment checks, including National Criminal History Check upon successful appointment
